Cognitive Health Assessment
Multimodal training for primary care teams to conduct a 5-10 minute cognitive health assessment (CHA), with information on additional resources for evaluation and care planning.
The CHA can be used whenever a cognitive assessment is indicated, including during the Medicare Annual Wellness Visit.
These are the 3 different ways you can take the cognitive health assessment trainings. By taking the training, learners will be able to bill for the assessment, including in patients with Medi-Cal only coverage using CPT code 1494F and receive FREE CE/CME.
Live Cognitive Health Assessment Training
Held on Zoom at least 1x monthly in partnership with the Alzheimer’s Association, open to anyone on the healthcare team.
Online Module
The online module is intended to be completed at the learners pace and can be completed anytime, anywhere.
