Cognitive Health Assessment
The Cognitive Health Assessment: The Basics is a multimodal training for primary care teams on how to conduct a 5–10-minute cognitive health assessment (CHA), It also includes information on the next immediate steps in an evaluation and starting a brain health plan.
The cognitive health assessment can be used whenever a cognitive assessment is indicated, including during the Medicare Annual Wellness Visit.

Here are three ways to complete the cognitive health assessment trainings, all offering FREE CE/CME. Billing providers in California can also bill for the assessment, including for patients with Medi-Cal only coverage, using CPT code 1494F.

Live Cognitive Health Assessment Training
Held on Zoom at least 1x monthly and, open to anyone on the healthcare team. Provided in partnership with the Alzheimer’s Association.

Online Module
The online module is intended to be completed at the learner’s pace and can be completed anytime, anywhere.
